Top 40 Under 40 Extended Questionnaire for Nominees

Please complete the following nomination form to officially submit your name for consideration for Top 40 Under 40 2021. We estimate that it will take 30 – 45 minutes to complete the form in full. You must complete the questionnaire in one sitting.

Your completed questionnaire must be submitted by May 31, 2021 to be considered for Top 40 Under 40 2021.

The information provided in these applications will be held in confidence by the editorial staff of Odvod Publishing/Edify magazine and the Top 40 judges.

    • Please list ALL post-secondary institutions, any accredited trade or professional development programs, any recognized academies attended and the names of the programs you have attended, including those where a diploma or degree was not granted. (For example, University of Alberta, BSc).

    • Briefly explain your company’s primary business. (If you are an artist, athlete or sole proprietor, consider your body of work your primary business and explain it here.) What do you do in your work role? Remember that the judges may not be familiar with your industry or field, so try to avoid jargon.

    • What impact have you made within your field in this role? Please note any specific growth numbers (e.g. revenue, client, employee or profit growth), publications or shows, or the start of groups or businesses. Try to avoid using percentages and instead provide specific absolute numbers. (Try to be as specific as possible. For example, provide numbers of clients or employees, growth in profit or completion of major initiatives.)
